Ist der SSI-handler überhaupt für diese Datei aktiviert?
Üblicherweise nennt man die Dateien .shtml (man beachte das
»s«) und aktiviert in der Apache-Config für .shtml den
SSI-Handler. Wenn man das vergißt, führt es zu obigem
Symptom.

Wenn Du das getan hast und es trotzdem nicht geht, zeig
bitte mal den relevanten Teil Deiner Config. Eine URL
zur Demonstration des Problems wäre auch nicht schlecht.
