Sieht komplett aus - zuerst dachte ich »da« fehlt, da du das woanders
stehen hast als im SCSI Part.

Packe mal USB_DEBUG mit in den Kernel rein und mache vor dem Einstecken
ein sysctl hw.usb.umass.debug=10

Kommen noch weitere Meldungen, die hier nicht stehen?
Zum loggen der letzten Zeilen kommt das System ja evtl gar nicht mehr.
